We're mobile! Our new patient check-in forms link will be sent to you via text message before your child's initial visit. However, if mobile check-in is not available to you, you can download/print paper copies to fill out before your child's first visit. This ensures a smooth and efficient first appointment for your family. 

If you’ve completed the paper forms, they can either be mailed (100 Genesee St., New Hartford, NY 13413) or brought with you to your appointment.
